Liability Waiver
Payment & Notification: Full registration for classes is not complete until payment arrangements have been made before the commencement of classes. There is a $50 fee charged on all NFS cheques. There are no refunds for missed lessons. If a student wishes to withdraw from lessons, a one-month notice must be given in writing for withdrawal.
Class Placement: Align BodyMind reserves the right to place students in the most appropriate level depending on age and/or ability.
I, the undersigned, do herby consent and agree that Align BodyMind, its employees, or agents have the right to take photographs, videotapes or digital recordings of my child in his or her dance class and any dance presentations and to use these in any and all media, now and hereafter known, and exclusively for the purpose of advertising and promotion. I do herby release Align BodyMind, it’s agents and employees all rights to exhibit this work in print and electronic form publicly or privately and to understand that there will be no financial or other remuneration for recording them, either for initial or subsequent transmission or playback in perpetuity.
Physical Contact Release: Dance is an art form that requires teachers to be able to have appropriate physical contact for the purpose of making technical corrections with participant consent. Consent is granted for such physical contact.
Injury Release: I herby release Align BodyMind, all program staff and/or persons involved with the direction and organization of Align BodyMInd activities from all claims for damages arising from any accident or injuries which are caused by or arise from participation by my child or children, names on this form, in any facility or at any location where Align BodyMind is being held. In the event of an accident or illness involving my child or children while attending Align BodyMind, I herby authorize, if I am not available, the administration of any medical procedure deemed necessary by the child or children’s physician selected by the program staff. I also give my permission for my child or children to be transported to the physician’s office or hospital, with no liability on the driver’s part. An ambulance may be called to transport my child or children to the hospital if required.
All programs and classes are subject to change pending registration.
